2009 CAYFL Registration Information
CAYFL uses a "
age / weight
" chart to establish our Tackle football teams.
Each Association will hold a minimum of three "on-Site" Registration Days.
These are scheduled for April 4th, May 9th, June 13th. from 9:00 AM to Noon , some Associations may be open longer.
Please check your Assciation web site for LOCATION and TIMES.
Many Associations will hold other dates, check on the Association web site for information.
Also, each Association has the chance to hold "On-Line" Registrations, right on their Association web site.

WHAT YOU WILL NEED TO REGISTER
1. Parent / Guardian of child to register
2. the player / cheerleader for official weight check, or to be fitted for football / cheer uniform
3. an "orginal" copy of the child's birth certificate, please bring a copy of this for the Association to keep !
4. Proof of Residence, this can be either a Electric / South West Gas / Water bill ONLY, please make a copy of it for the Association to keep on file.
5. the child's "final" 2008 - 2009 Report Card
6. payment of
all
Registration Fees
ALL PARTICIPANTS MUST HAVE THEIR "
MEDICAL CLEARANCE
" BEFORE THEY CAN BEGIN TO PRACTICE !!
just click on the "handouts / forms" page to download a copy of this form. It can be compleated by your medical provider, and returned to your Association as soon as possible. ALL Tackle / Flag and Cheerleaders
MUST
have this form turned into your Association, before they begin practices.
